Bloomington home care agency cited for neglect
The state's department of health has cited a Bloomington home care agency for neglect, after an investigation found that one client slept on a urine-soaked mattress and other clients had their needs ignored.
The department opened an investigation into Destiny Home Care Services, Inc. on Sept. 10, 2009, after receiving a complaint that the agency's clients were unshaven, had dirty fingernails, wore dirty clothing, and smelled of urine.
Investigators visited the agency's three residential sites in Bloomington and released their report last week.

- One client with incontinence problems had a mattress "saturated with urine."
- A client with a criminal history of molesting young boys had no agency plan in place to prevent her from engaging in illegal sexual behavior.
- Staff members took a client with severe, uncontrolled diabetes to McDonalds on three days when the client's blood sugar was elevated.
- A nurse noted that another client with diabetes had an unhealthy diet consisting only of "Burger King and pop." The facility has no records of the nurse addressing these concerns.
- A female client told staff members that she had been raped, but the facility did not report the allegation to any local authorities.
The department of health also issued a $1050 fine for medication and training violations.
The agency did not immediately respond to a request for comment.
